Right atrial ball thrombus associated with atrial septal occluder device: A late complication of transcatheter atrial septal defect closure

چکیده
In a large autopsy study, intracardiac ball thrombi are usually seen in the left atrium and rarely seen in the right atrium. They may be one or more in number. Right atrial ball thrombus may appear as a complication of central venous catheterization or be coexistent with tricuspid stenosis. A 42-year-old woman who began routine hemodialysis 5 years ago due to chronic kidney disease was admitted to our hospital because of dyspnea. She had undergone secundum atrial septal defect closure with Amplatzer septal occluder (ASO) device 7 years ago and had not received antiplatelet or anticoagulation therapy. She had a normal sinus rhythm. Her physical examination at admission revealed normal results, except for a thrill on the right wrist due to an arteriovenous fistula. Transthoracic echocardiography revealed a large, ball-like thrombus in the right atrium associated with the atrial septal occluder device (Fig. 1, Video 1). The thrombus was 22x22 mm in size, and the morphology of tricuspid valve was normal. The tricuspid regurgitation jet was moderate with a velocity of 3.6 m/s. No residual defect of the septal occluder device was observed. Computed tomography angiography was performed for pulmonary embolism screening, and subsegmental filling defects were detected. In addition, ball-like filling defect adjacent to the ASO device was clearly observed in the right atrium on computed tomography images (Fig. 2). To the best of our knowledge, this is the first case report of right atrial ball thrombus associated with the atrial septal occluder device. Video 1. Urgent Surgical Intervention for Embolized Cardiac Occluder Devices: A Case Series
Introduction: In this study, we sought to illustrate our experience in urgent surgical management for embolized cardiac septal occlude devices resulting from trans-catheter closure of atrial septal defect and ventricular septal defect. Mathrials and Methods: We retrospectively reviewed four patients aged 2–10 years who underwent urgent surgery due to cardiac septal occluder embolization between...

Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...
متن کاملSuccessful treatment of left atrial disk thrombus on an Amplatzer atrial septal defect occluder with abciximab and heparin.
Acute device thrombosis is a rare but important complication after transcatheter atrial septal defect closure. In this case a mobile thrombus was noted on the left side of an Amplatzer atrial septal occluder after device release in a 12 year old boy with an uncomplicated atrial septal defect. The thrombus was successfully treated with an infusion of heparin and the glycoprotein IIa/IIIb recepto...
متن کاملCardiac perforation after device closure of atrial septal defects with the Amplatzer septal occluder.
OBJECTIVES Amplatzer septal occluder (ASO)-associated cardiac perforation (CP) at our institution prompted this retrospective review. BACKGROUND Cardiac perforation is a rare complication after transcatheter atrial septal defect (ASD) closure. METHODS To identify CP after transcatheter ASD closure with ASO, cardiac events (CE) describing definite CP, hemopericardium, pericardial effusion, c...
متن کاملA Silent and Late Embolization of Atrial Septal Defect Occluder Device Into the Right Pulmonary Artery: A Case Report
Percutaneous device closure of atrial septal defect (ASD) is an alternative treatment to surgery. The main advantages of the percutaneous approach include avoidance of surgery, short procedure time and hospital length, in addition to comparable rates of complications. However, percutaneous device closure is associated with infrequent early and late complications including device embolization, a...
